Jack Duane Forman

Jack Duane Forman, 77 of Arthurdale, WV, passed away on Tuesday, August 30, 2022 at Ruby Memorial Hospital with family by his side. He was born on October 15, 1944 in Arthurdale, son of the late Junior Ralph and Jettie Iretta (Boggs) Forman. Jack was owner and operator of his own company, Jack D Forman Trucking. He loved watching westerns and was known as John Wayne. He enjoyed old trains, being a heavy equipment operator and digging in the dirt. He loved his family. Jack is survived by his loving wife of fifty-nine years, Mary Katherine (Smith) Forman; three daughters, Jackie Kaye Bennett and James, Reedsville, Marcy Lynn Koontz and Jack, Arthurdale, Misty Raye Hawkins and Jeremy, Arthurdale; eight grandchildren, Nathan Forman and Sarah, Masontown, Joshua Shrout, Masontown, Ashley Shrout, Arthurdale, Felicia Beshears and Aaron, Independence, Levi Koontz, Masontown, Payton Koontz, Arthurdale, Jaden Hawkins and Bryson Hawkins, Arthurdale; five great-grandchildren; Cameron, Liam, Landon, Trinity and Bentley; one brother, James R Forman and JoAnn, Arthurdale; one sister, Iretta Jean Turnley and Ed, Reedsville; one adopted son, Tim Turner; one special friend, Rick Maczko; his special fur-babies, Ellie and Zoey; and several nieces and nephews. In addition to his parents, Jack was preceded in death by two grandchildren, Jordan and Johnna Koontz.  In lieu of flowers memorial donation can be made to WV Caring PO Box 760 Arthurdale WV 26520.
